In certain situations, we may share your information without written permission as required by law, for public health purposes, to report abuse or neglect, to address health oversight activities, in judicial proceedings, for law enforcement purposes, to coroners or medical exameners, and to avert a serious threat to health or safety.
Special Protections
Certain types of information, such as psychotherapy notes, substance use disorder records, and HIV-related information, receive additional legal protections. We will obtain your specific written permission before sharing these records except in limited circumstances permitted by law.

Filing a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us and with the Secretary of the Department of Health and Human Services.
